Breast implant placement options

Breast implants are placed behind the mammary gland in four main breast implant placement locations:

  • Subglandular: The implant is positioned between the breast tissue and the pectoralis major muscle. Pectoralis major is the large muscle of the chest that lies under the breast. This breast augmentation placement option is considered by many to achieve the most aesthetic results and natural look. Unfortunately the sublandular position is usually not recommended for women with thin subcatenous tissue or defective deep skin tissue, which may be manifested by stretch marks. Capsular contracture and wrinkling rates are slightly higer if this implant location is selected for your breast implant surgery.
  • Subfascial: The implant is positioned similiarly to the subglandular position but underneath the fascia of the pectoralis major. This position reduces the wrinkling and results in additional soft tissue coverage for the implant. Additionally, the implant edge visibility and palpability, common with subglandular placement is reduced.
  • Subpectoral: The breast implants are placed behind the breast tissue and partially under pectoralis major. Inferior muscular attachments are released. This technique is the most common in the United States and results in very good coverage for upper part of the implant. The incidence of capsular contracture and wrinkling is much lower. There is also a down side to this approach - less cleavage and limited natural breast movement.
  • Submuscular: The implant is positioned similiarly to the subpectoral position, but the outer and lower edges of the implant are covered by the serratus muscles and fascia that connects pectoralis major, serratus, and upper rectus abdominal muscles. This technique is most commonly used in breast reconstruction. Submuscular placement has the least risk of wrinkling, bottoming out, and interference with mammogram readings.
